A Vaccine Information Statement (VIS) is a one-page (two sided) information sheet, produced by the CDC.  VISs inform vaccine recipients about the benefits and risks of a vaccine.  Please read the Vaccine Information Statements below, prior to your child’s upcoming well visit.
